21 Jan 1945
USS Maddox (Cdr. J.S. Willis) is hit by a Japanese kamikaze aircraft east of Formosa in position 23°06'N, 122°43'E. 7 of the crew were killed and 35 were wounded in this attack of which one died the following day. Maddox returned to Ulithi for repairs.

14 Mar 1945
USS Maddox (Cdr. Selby Krauss Santmyers) departed from Ulithi to participate in the operations of Okinawa.
